This policy explains how we collect, use, and safeguard your information when you use the App. By downloading, installing, or using the App, you agree to this Privacy Policy and our Terms and Conditions, incorporated herein by reference. To use the App, you must read the Terms and Conditions and Privacy Policy and acknowledge your acceptance or agreement. If you do not agree, you must uninstall the App. Please read this policy carefully to make informed decisions. “Services” are to include our website, App, platform and services provided (collectively, “Services”).

  • Patients/Users: Individuals who use our App and Services to manage their health information and choose to share it with their healthcare providers.
  • Healthcare Practitioners/Providers: Professionals who use our App and Services to access information shared by their patients and utilize our data analysis tools.

What Information Do We Collect?

  • Information Patients Provide: When patients use our App and Services, they may provide personal information such as name, email address, date of birth, and health-related information they choose to input or connect to our App and Services.
  • Information Practitioners Provide: When practitioners use our App and Services, they may provide professional details, information about their practice, and information related to the patients they enroll or interact with through our App and Services (with appropriate consents where necessary).
  • Information from Your Use of Our Services: Like most online services, we may automatically collect technical information when you interact with our website or platform. This might include your IP address, browser type, operating system, geolocation data, payment information, and how you navigate our App and Services. We use this to maintain and improve our App and Services.

How Do We Use Your Information?

We use your information to:

  • Provide and Improve Our Services: To operate our App and Services, enable its features, and enhance your user experience.
  • Facilitate Patient-Controlled Sharing: A core part of our Service is to allow patients to share their health information with their designated healthcare provider(s) through our App. This sharing only happens if the patient actively chooses to allow it.
  • Enable Data Analysis for Practitioners: We work with practitioners to analyze the data of patients who have consented to share their information with that practitioner via our service. This helps practitioners gain insights to support patient care.
  • Communicate With You: To send service-related updates, security and service alerts, support messages, and (if you opt-in) information about new features or services.
  • Process Payments: If applicable, for any subscription fees or service charges.
  • Maintain Security and Compliance: To protect our App and Services, enforce our terms of service, and meet legal and regulatory requirements, including those related to HIPAA.

Will We Share Your Information?

Your trust is our priority. We do not sell or give away your personal information for third-party marketing purposes without your express permission.

We share information in the following ways:

  • Patient-Directed Sharing with Providers: If you are a patient, your health information will be shared with your healthcare provider(s) through our App only when you explicitly choose to do so as part of using our Services. You control this sharing.
  • With Healthcare Practitioners (for their Patients): If you are a practitioner, you will be able to access information for those patients who have chosen to share their data with you via our Services, and utilize our tools to analyze this data.
  • With Third-Party Vendors: We may share information with trusted third-party companies that help us provide and improve our Services (e.g., cloud hosting, data security services, payment processors). These vendors are contractually obligated to protect your information, use it only for the services they are providing to us, and adhere to HIPAA security standards where applicable.
  • As Required by Law or to Protect Rights: We may disclose information if we believe it’s necessary to comply with a law, regulation, legal process, or governmental request; to protect the safety of any person; to address fraud, security, or technical issues; or to protect our rights or property or the rights or property of those who use our Services.  
  • Business Transfers: Data may be transferred in corporate sales, mergers, or similar events.
  • With Your Explicit Consent: For any other purpose not listed here, we will ask for your specific permission before sharing your identifiable personal information.

How Do We Protect Your Information?

We take the security of all information on our App and Services very seriously, whether it’s patient data or practitioner data. We use HIPAA-compliant security protocols and safeguards to protect your data. These include:

  • Technical Safeguards: Such as encryption (to protect data in transit and at rest), access controls, firewalls, and secure server infrastructure.
  • Administrative Safeguards: We have policies and procedures that limit access to information to authorized personnel who need it to perform their job duties. We also train our staff on data privacy and security. You are responsible for securing your device with up-to-date antivirus software and strong passwords.
  • Physical Safeguards: Measures to protect our physical facilities and equipment where information might be stored or accessed.

While we strive to use commercially acceptable means to protect your information, no method of transmission over the Internet or method of electronic storage is 100% secure.   

Your Rights and Choices Regarding Your Information

  • For Patients:
    • Control Sharing: You control whether to share your information with your healthcare providers through our service. You can disable certain information sharing such as geolocation data or push notifications through the App and through your device.
    • Access and Correction: Much of the health information managed through our service originates from you or your provider. You generally have the right to access and request correction of your health information through your healthcare provider, who is the covered entity under HIPAA. You can also typically review and update the information you have directly provided to our service through your account settings.
  • For Practitioners:
    • You can manage your account information and the information related to patients you enroll (with their consent) as per the functionalities of our service.

We may use cookies (small text files stored on your device) and similar technologies to help operate our Services, understand usage, and improve your experience. You can usually manage your cookie preferences through your browser settings.

Children’s Privacy

Our Services are not directed to individuals under the age of 13 (or a different age if specified by applicable state law for health information consent without parental involvement). We do not knowingly collect personal information from children without appropriate parental or guardian consent, or as directed by a healthcare provider using our service with the necessary authorizations. If we become aware that we have collected personal information from a child in violation of applicable law, we will take steps to delete that information.  

Data Retention

We retain your information for as long as necessary for App functionality, to provide our Services to you and our practitioner clients, or as required by our contractual obligations with practitioners and applicable laws. You are responsible for backing up health data stored locally on your device.

Changes to This Privacy Policy

We may update this privacy policy from time to time. If we make significant changes, we will notify you through our App, Service or by other means (like email or our website) and update the “Effective Date” at the top. We encourage you to review this policy periodically.
